Are you a talented individual, ready to work within our newly formed CHC Hub?

We will look to you to inform decision making and provide expertise regarding Continuing Health Care. Providing innovative and effective services to improve the lives of the individuals we work with.

The team currently works across health and social care services based at Poole Civic Centre working across Bournemouth and Poole, maintaining our strong established partnerships, recognising both the challenges and benefits this can bring.

The team works jointly with the Clinical Commissioning Group in completing Multi-disciplinary assessments to determine eligibility for CHC. The team also offers support to Bournemouth and Poole’s community teams in completing the CHC process. A strong knowledge in Continuing Health Care is essential as you will be required to complete assessments for complex cases and with service users who have complex needs.
