An exciting opportunity has arisen for a Qualified Social Worker to join our busy Sight and Hearing Team within the community.
The Borough of Poole’s Adult Social Care Services Sight and Hearing Locality Team aims to provide a quality service to adults over 18 who have a Sensory impairment.
We are looking to appoint a Social Worker to work in our motivated and enthusiastic team.
The role will involve undertaking comprehensive assessments focusing on the needs of Clients and Carers, supporting and enabling people to minimise dependence and support independence through self directed support, commissioning, monitoring and reviewing packages of care to meet individual assessed needs. The role will also involve the demonstration and suppling of Equipment that can support Sensory impairments.
You will be based within an integrated multidisciplinary team delivering outcome based services. You will be expected to demonstrate excellent communications skills, advocating for adults to assist them to achieve their outcomes, including signposting and providing information and advice.
This will be a part time post, working 18.5 hours per week.
